As the backbone of this work, Close to Home drives system-wide planning, performance, and accountability to ensure resources are used effectively and progress is measurable. Through data-driven decision-making and community-wide collaboration, we work to deliver lasting, system-level solutions that make homelessness rare, brief, and non-recurring.

We track key indicators through a community-wide performance monitoring framework to understand progress, identify challenges, and guide decision-making.
