As the backbone of this work, Close to Home drives system-wide planning, performance, and accountability to ensure resources are used effectively and progress is measurable. Measuring What Matters

We track key indicators through a community-wide performance monitoring framework to understand progress, identify challenges, and guide decision-making. Our quarterly evaluations of the strategic plan bring together both quantitative data and qualitative insights through our Pathways to Progress initiative. 

This approach ensures we are measuring progress, learning from partners and lived experience, and continuously improving. By grounding decisions in both data and real-world insights, we help focus resources where they can have the greatest impact and ensure strategies evolve based on system performance and community needs.

Prevention

We work to reduce inflow into homelessness by expanding targeted prevention and diversion efforts and strengthening a coordinated, no wrong door approach.

Crisis Response

We strengthen outreach and shelter systems to ensure people experiencing homelessness can quickly access support and pathways to housing. 

Housing Solutions

We expand access to safe, stable housing by scaling permanent housing options and improving pathways to long-term stability.
